"Application for self-service recovery" Newbie Question

I've attempted twice to recover some VET coins I withdrew from Binance and sent to an Ethereum wallet (Meta Mask). When I submit the form I get this error message:

"The currency, network, or amount of the TxID/TxHash do not match your input. Please confirm the details provided."

These are the 5 fields in the form.. The only thing I can guess is that the (Deposit Network) field is filled in wrong and should be Ethereum, If so, is ETH, Ether or Ethereum the correct entry? If not, any help is appreciated.
